EKGs look electrically at the heart at different angles. The doctors needed to know which "view" of the heart they were looking at.

Lead 2 is the "main lead" we look at, it gives a decent front on view of the heart, at least enough of one to ID death rhythms. Each other lead is just a view around the heart.

Lead 2 is commonly the limb lead showing P waves the best, thus differentiating between normal sinus rhythm and other rhythms such as atrial fibrillation, junctional rhythm etc.

Not to go off topic (maybe this should be one of its own), how many ems departments out there are transmitting to the ER you monitor readings? We have been transmitting for the last 3 years to a few of our hospitals, now almost all of them have the capability to get them. Since we got our new Lifepak 15's a little over a year ago we do it a lot more often.

We transmit 12-Leads on every cardiac chest pain pt. Not only does it go to the ER, but it auto forwards to the cath lab and a couple of other places in the hospital. We are having ER times of less than 5 minutes before the pt is on their way to the cath lab.

Also, just to clarify, even you 30+ plus y/o cardiac monitors show lead 2. Leads 1, 2, and 3 are the most used. Only if we find something wrong (or can't find anything wrong in some cases) will we use a 12 lead. My full time service has been sending EKGs for several years now, and recently had a camera installed in the patient compartment that we can transmit to the ED as well. So far, both cardiac patients that I've transported (only been here for two months) have by-passed the ER in favor of the Cath Lab.

none of the ambulance services around here have the ability to transmit EKG, atleast if they do, they dont transmit..
