Kerry, yours look very odd.
It seems that the long lamp holders are mixed up with the short lamp holders so you have got tilted sealed beams.
You have two down tilted and two up tilted.
Intentional or unintentional?

If I recall the top mounting plates were reversed which tilted the bulbs. After I corrected it the bulbs were back to facing straight out. I had another light that came thru like that also. Im not sure if the previous owner was trying to make it a hill light or just didnt have the top brackets on correctly. Id check the light more closely but its outside right now in my frigid cold garage so I'll have to pass....LOL

I dont know any way to restore name plates. Once the fade out there isnt much you can do. I tried to clean them up already and you usually end up making them worse. When I buy a light I try to get ones with decent ID labels. Im kinda a stickler for that.
